This Employee Performance Appraisal Form is a formula-driven Excel template built for HR teams, people managers, and small business owners who need a structured, defensible way to evaluate employee performance without relying on a subjective, unweighted checklist. Instead of a single overall impression, this form separates performance into what an employee achieved and how they achieved it, then combines both into one final, calculated rating.

The form opens with Employee & Review Details, then moves into Goal/Objective Achievement, where up to several goals are weighted by importance, scored against actual results, and automatically rolled into a Total Weighted Goal Score out of 5. Competency Ratings follows the same weighted approach for behavioral and skill-based factors — communication, teamwork, job knowledge, problem solving, initiative, punctuality, and leadership — producing a Total Weighted Competency Score. Both sections include a built-in weight check confirming each set of weights sums to 100%, catching a common scoring error before it skews the final result.

The Overall Performance Score section combines the Goals and Competency scores using a configurable weighting (60/40 by default, adjustable to match your organization's appraisal policy), and automatically looks up the corresponding rating band — Outstanding, Exceeds Expectations, Meets Expectations, Needs Improvement, or Unsatisfactory — removing any ambiguity about where a numeric score falls. The form continues with structured space for strengths, areas for improvement, a development plan, employee and manager comments, and a formal sign-off section for employee, manager, and HR signatures, making it usable as a complete, ready-to-print appraisal document rather than just a scoring worksheet.
